Call us at (940) 891-3600 or use our contact form to schedule. We accept most major insurance plans and offer flexible scheduling including evening and weekend appointments.
Complete Intake Forms
New patients can fill out forms online before their visit or at the clinic. Please bring a valid ID, insurance card, and a list of current medications and allergies.
Your First Visit
Plan for approximately 45–60 minutes. We'll review your medical history, discuss your health goals, and perform any recommended screenings. Ask questions — we're here for you.
Follow-Up & Ongoing Care
After your visit, we'll outline a personalized care plan. Our team is available between appointments for any concerns, and we'll remind you when annual exams are due.
